I'm not in a position to interpret your proxy server logs, but it looks like a simple authentication failure.  My suggestion about taking a packet trace stands.  In situations like this I generally recommend that you take a packet trace of a working client (your NSURLDownload code, for example) and compare it to that of your failing client.

btw It'd help if you replied to the list rather than directly to me.
