Are you by chance generating long-GOP material, like XDCAM or HDV? I experienced a similar issue once, which ended up being related to the chunk alignment in the MOV file being created. I think it had something to do with the video and audio offsets in the header, with the chunk sizes being too big. Unfortunately I am not a developer and don't understand this well enough to get into specifics. Hope this helps!
