Did you try throwing out the original spreadsheet and then re-downloading? Once you have the new copy, apply again to the iPads.

I think I'm screwing up something simple, tried VPP support by email but
they pointed me to enterprise support and I spent 30+mins on hold and gave
up.
Apple's instructions (http://support.apple.com/kb/HT5188) state to
download a paid app to itunes using 1 redemption code from the spreadsheet.
I did this and applied the app to 10 ipads out of 20 codes (using Apple
Configurator), but it shows 11 codes used (1 for itunes on the mac).
The Apple doc above states to re-download the spreadsheet and re-import,
but ours just shows 11 codes are used.
How do you free up or apply this 1 floating code so it can be used on an
iPad?
