Begin your url with "jdbc:mysql://". The Username and Password will be
set for your database by the db administrator. They can be set on a
global or database (I think these are called catalogues in SQL) level,
so that really depends on your settings.
We have never filled the Driver and Plugin fields.
We tend to use the latest stable drivers. We test them in development
environments and when happy use in deployment. They tend to have
performance improvements. Keep a copy of the old, working driver just
in case.
