I wasn't using the addObjectTo/removeObjectFromBothSidesOfRelationship... but did change to using those functions and I still get the same result.  I am not using pessimistic locking and have checked the delete rules and they appear to be correct.  I did not change them from what EOModeler set them to when I chose join in many to many from the menu to connect the two tables item and option.  What else should I be looking at.
