I'm old skool...I do it all by hand. That way I don't get any fluff in the HTML, and it does what I want in a way I understand it.

And since I learnt CSS and table-free layouts, the HTML has become much smaller and simpler, and the variety I get reusing the same components and only tweaking stylesheets is amazing.

I guess you could get all that with DReamWeaver and GoLive too, but I see them as being page-centric as opposed to component centric.
