I'm quite sure one of the following will help:

1. Check the EOModel's connection dictionary: IP address correct? Username/password? My connection parameters for MySQL should look like this: jdbc:mysql://127.0.0.1:3306/<db-name>? capitalizeTableNames=true&autoReconnect=true

2. Check for missing frameworks: JavaJDBCAdaptor.frameworks is needed on the classpath MySQL JDBC adaptor is needed on the classpath If you're deploying on a Mac you can check the Apps's classpath in: <appname>.woa/Contents/MacOS/MacOSClassPath.txt or MacOSXServerClassPath.txt
