James: If you're looking to have 100% up-time, you may want to consider 2 application servers (of course, a minimum of 2 each is great, i.e. 2web/2db/2app). Then, if you need to install an update, you can divert traffic from one server to the another, install your update, reverse the process and then finally resume standard traffic patterns. You can always add a server down the road if necessary.
