I'm a java client user. I have seen this exception from time to time. I have found that if EOEditingContext.saveChanges() fails, it's often hard to recover on the client without restarting the client. It would appear that state gets updated on the server as if EOs were saved, but they don't appear saved to the client, or something like that. In any case, the server and client do appear to get out of sync and I haven't spent a lot of time figuring out how to gracefully recover in these cases...
