To overcome this annoying intermittent and unexplainable behaviour in
about 10 seconds, do this:
Drag the files directly from the Finder to the XCode Project "Groups &
Files" pane. XCode will present the customer options for adding a file
to the project .... continue from there.

I've had this happen before, but can't remember (or never knew) what
to do to resolve it.
Does anyone have an idea of what the problem migh be?
