If it is a publishing app with very little user-write interaction (like a news site) then it may be possible to use XML/XSLT and create a stylesheet for each appearance. This has worked well on one site I have worked on. I have covered this in an article a couple of years ago...

I might do a couple of things a bit differently now-days, but the concept is still the same.

Generally though, if you can get away with swapping out the CSS based on the site, I would do that as it is far less time consuming in my opinion.
