+1. I also want this problem with one of our sites (www.acaiq.com),
and it always some kind of desktop apps that do strange things to
URL. It can be found by using the "combined" log format in Apache so
that you can look at the user agent used when the error was throw.
If you're seeing it on-screen after clicking on something, it's
probably a badly formed hyperlink.
