I think I got it.

It turns out the app was exiting almost immediately. Auto Recover was kicking the app back on without me knowing it was even turned off. I changed a directory path (the app was exiting if it couldn't write a file to it) and now and now it seems to work properly. I never would have thought to look for that except I kept seeing error message blips in the application logs I had open. They would disappear as soon as Auto Recover turned the app back on.

Thanks for all great assistance. This is what I call a Developer Community!

Yes. I have my domain in the Domain Name field. Everything else is the same.
I also have a line with Domain Name: localhost, IP Address: Any, Port: 80

Was ON. Is now OFF for both lines

I will try everything again now that performance caching is turned off.
