With the original deployment, here is what happens :

Don't remove the any of the files from their locations. Servlet deployments are not as simple as they look.
Start over with your deployment, don't make any changes to the web.xml file and don't make any changes to the file structure expanded out of the .war file.
What happens when you do that?
