Yes.

Ideally there should be NOTHING in /Library/Java/Extensions.
Yes.

Slightly less ideal is to have JDBC drivers in there.
Yes.

mail.jar and activate.jar if you must.
No.

Pretty much anything else is just asking for classloader problems (which is what you have been experiencing).
