ERModernLook and ERMODPickListPage

Dear All,
While using ERModernLook, when I select ERMODPickListPage template, I get following output. (Ignore the white space this is just the hidden content).
But when I use ERMODPickListPage template with ERMODListTemplate (using Chuck's API to inherit template from ERMODListPage) I get the right output. But I can't show Select All, Clear Selection buttons as they are not part of ERMODListPage template.
Is there anything I'm missing to make the ERMODPickListPage to show the top navigation bar and paginator at the bottom as I want to achieve something following which appears with ERD2WPickListPage.
