Though I've pored over my code, I'm sure that the session not getting checked in is the cause too. This sort of thing has cropped up before and it has always been due to human error.
I'm still using 5.3. awake, sleep and terminate are overridden, but only to emit log messages. Those were after super, but I put them before in try . . . finally blocks. I didn't have the logging on, but will now. I call terminate, but only in a logout action. Incidentally, session terminated with that message seem to go away fine (actually most of the sessions do, except for these occasional problem ones).
I will refocus on the logs now that I'm sure it is something wrong with the session not being put to sleep correctly.
