Hand-coded HTML+CSS in Eclipse along with FireBug for CSS tweaking.  The WYSIWYG editors like DreamWeaver aren't very useful in my opinion.  If I need a really dynamic AJAXy page, I use GWT.  But I pity you if you try to learn WO and GWT at the same time.
