But what about the case where it works fine with the Gimp? When I launch the Gimp, 3 windows automatically pop up and they cascade properly. Surely the Gimp must be positioning these windows since X11 doesn't seem to be cascading windows. Or is this somehow another exception?

no, quartz-wm is cascading these windows. It's placement algorithm is currently:


1. if USPosition is set, use it
2. if WM_TRANSIENT_FOR == root window, centre the window on the display containing the topmost window in its group
3. if WM_TRANSIENT_FOR is set, but not the root, centre the window on the dialog's parent
4. find the topmost window in the same group, and cascade from it
5. if no other group members, place the window in the top-left corner of the main display
