I have had a similar problem on my Ti Powerbook G4 (400 MHz, 640 Mb) for
over a year, but I don't believe the behavior is restricted to X11. The
only dual display I have ever used was with an LCD projector, and I
haven't done that for at least a year. This problem has persisted since
upgrading from OS 10.2 up through 10.3.5. I have a display that is
perpetually off-screen (and therefore invisible); it can be moved
around, so I currently have it located at the bottom right corner of my
visible display.

If I save files to my Desktop, and the Desktop is fairly full, it will
start saving the icons onto the invisible Desktop, which makes it a pain
to track them down.
I had someone from Apple try to solve the problem a year ago after I
upgraded to 10.3, without success.
