quartzwm is probably what restricts the size to one monitor.
I can't actually test this myself, as I only have one monitor.
My boss has two at home and is the one who reported this
to me.

I'm not sure I understand the question, because right this moment I'm
looking at a window that spans two monitors in X11.
I have two monitors. The primary one, and a second one physically
sitting to the right. In the Monitors System Pref I have Mirroring
turned off and I have the second monitor positioned to the right. I
have not installed Xinerama; I don't even know what Xinerama is.
In X11 I open an emacs window, make it big. Then I drag it to the
right. I now see half the window on the main monitor, and half the
window in the second monitor. I've done the same with an xterm
window. Is that what you mean?
I am not, however, using quartz-wm, if that matters.
