You should be good to go as far as the right DISPLAY now.
Additionally, you are using com.apple.x11 when you should be using
org.x.x11 for defaults.
Also, the path is /usr/X11/bin/Xephyr, not /usr/X11R6/... (it's a
symlink now for backwards compatability, but you are encouraged to
use /usr/X11).
Why don't you try using startx to handle all of this startup stuff
instead of reinventing the wheel?
This works for me (well, after removing /usr/X11/share/X11/xkb):
startx /usr/X11/bin/twm -- /usr/X11/bin/Xephyr :2

Yes, it was set in my ~/.tcshrc.
That's been removed now.
AFter reboot, on 1st login @ X11.app's xterm, I now see,
echo $DISPLAY
/tmp/launch-zPNbqI/:0
which is different, but don't know if it's better?
either way, the errors with Xephyr, as reported above are,
unfortunately, still there and the same.
