Is this Tck/Tk with the X11 renderer, or Tcl/Tk with Aqua rendering?
If it's the former, it would be worth seeing how it works in the
latter case.

That was an issue with X in the original Leopard release. Have you
updated to the latest release?
This is with the latest 2.1.2 release candidate (2.1.2-rc6), its only
with tcl/tk apps though - horizontal scrolling works without issue in
the GIMP, for example.
