OK, I'm scratching my head here. I just installed 2.1.1 over 2.1.2 and am still seeing the problem. Now I'm wondering if I actually used the Cadence software with the offending toolbar while I had 2.1.1 installed (or maybe the down grade from 2.1.2 to 2.1.1 didn't overwrite all the 2.1.2 stuff?).
