This is roughly correct. xterm is using fontconfig for TrueType fonts, and
to the extent that fontconfig supports glyph substitution, it must be
configured separately. I have a partial configuration that I use which
improves things somewhat by adding various missing fonts, but have not
looked to see what support there is for glyph substitution in the way the
native framework does it. [...]
Just one tiny alternative approach.
While I'm mostly using iterm2 (Aqua app) on my Mac these days, on X11 I used to
use rxvt-unicode (executable name urxvt), which had as an explicit feature font
fallback to try to get good Unicode coverage.
You could install rxvt-unicode and see if it does better presentation for you.
