Strange menu key handling during debugging.

When the debug window is front most, the keyboard equivalents of Continue,
'Step over', 'Step into' etc are not recognized unless the focus is on the
thread view.

i.e. if you click in the Variables or Editor view they stop working.
Selecting the equivalent items from the Debug menu works in all cases.

On further inspection it seems as though this is only the case for some
keyboard equivalents. E.g. Control-s for Step Over.
