Yes, I've seen it several times. I have not been able to provide a
reproducible case, so I have not filed a bug report yet (at least I
don't think I did).
It's IMO not uncommon that Xcode becomes really confused with regards
to mouse events. Most of the time I think it's because it's main thread
is choked for such long periods of time that they are either dropped,
or somehow ends up out of sync - but that's really just a wild guess.
I can usually solve an issue like this one by switching to some other
application and back again.
