Xcode Build issues

Using XCode 2.1 I've found on many occasions that when I make changes to a source file in my project then Build and Run, the changes I've made don't make it into the new dev build. I spent ages going around in circles until I realised that all my changes where not going in to the new build.

I assumed the process of development was to make a change, then Cmd-R to Build and Run. Now it seems that's not the case (or maybe I have a setting not right somewhere?)

I discovered in the Contextual Menu an 'Update' command that I can find no documentation about. Applying this first before a Build and Run seems to work fine, but it's frustrating to have to select this each time and I can't find it on any menubar, with a shortcut key to speed up the process!
