I believe that it only acts on an editor pane. i.e., if an editor window showing a .cpp file has focus, it'll be available, or if you have the editor pane showing in the project window and you click on a .h file so that it shows in the editor, it will be available.

Codewarrior has exhaustive support to bind *any* action to a key
command or
toolbar button. In CW, I'm switching back and fourth between .h
and .c/.cpp
files all the time, so if I have to reach over and grab the mouse
every time

Haven't seen anyone else answer this one, so: Cmd-Option-Up Arrow does this. At least that's what does it on my config. It's under the View menu (Switch to Header/Source File), so you can bind whatever you want to it in the Prefs panel -> Key Bindings.
