I had the same problem in 2.0. The ghost breakpoints were set from another project from the one I was using to debug the application. Once I opened that other project, I could see the breakpoints and remove them.

I can set a breakpoint just fine (XCode 2.1). However sometimes after
I delete a breakpoint (it is visually deleted and does not show up in
the breakpoints window) my code keeps stopping at that point!

I've tried everything and there seems to be some breakpoints that
just won't go away.

Where are the breakpoints stored? Perhaps if I delete where they are
kept I can finally get rid of these phantoms. Note my code consists
of multiple projects/executables/libraries/plugins.
