It turns out that you need to change the definitions of CC and CXX in
$PREFIX/build/config_vars.mk to be g++ rather than gcc (where $PREFIX
is wherever you installed Apache, e.g. /usr/local/apache).
With that, it all works nicely with the Xcode 2.2 tools.

BTW: The libtool in question is a GNU libtool, but it isn't the
Apple supplied libtool, it comes with the apache distribution.

ld doesn't know about either C or C++. More specifically, it
doesn't know which files the compiler needs for all of the various
options which can be passed. That's why linking with gcc or g++
is recommended (and I think libtool typically does exactly that).
I guess I can edit the libtool script, but then I'm not sure I'll
be able to compile C-only modules anymore!
They'll work. You can use g++ to build C code; it'll just link in
some unnecessary C++ stuff.
