This might help with making your own .pbfilespec and .pblangspec.

I'm currently trying to figure out how to get custom icons for files
in Xcode.  So far I've gotten to show the icon if you go Get Info ->
Open With -> Other and then just choose Xcode (even though the
default is already there as Xcode).  That causes the icon to show up,
but if you do Change All the icon goes back to the plain white document
icon.

As for File and Project Templates, they are in /Library/Application Support/Apple/Developer Tools/. For File Templates, go to one of the simple file templates like BSD/C.pbfiletemplate and try to mimic that. For Project Templates, make an Xcode Project and put in the basics of what you want and then put in there under the category you want.
