Joining this thread (very) late:

I am seeing this on my machine as well (PowerMac G5, Mac OS X 10.4.8, iTunes 7.0.2), and it does seem correlated to activity (compiling, launching, switching from one app to another, etc) but doesn't correlate with the intensity of activity (just switching apps will sometimes cause it). Not sure, but I think it started after I upgraded to Mac OS X 10.4.8.

And it isn't limited to iTunes. QuickTime Player does the same thing. And not just music files. Playing internet streams stutters as well, in both iTunes and QuickTime player.
