Separate editor windows pref not working

I have my preferences set to use separate windows, and that's been working
for months. But the past couple of days, when I add new files to my project,
they create windows that mess this up. If I add foo.cp and foo.h, then when
I open foo.h, not only does clicking the little "Go to Counterpart" icon
open foo.cp in the same editor window, but as long as that freakin' window
is frontmost then double-clicking any other file in the tree opens it in
that window, and even right-clicking any other file and choosing "Open in
Separate Editor" opens it in that window.

I'm assuming there's some corrupted pref somewhere that need resetting, but
where? I've tried going to XCode preferences, turning off separate editors,
applying, turning them back on, and applying, but that didn't fix it. Is
there some pref that is file or window specific?
