I've had a similar problem where breakpoints I've removed come back
the next time I quit and launch Xcode.
I think it's related to deleting or moving a function that contains
breakpoints or setting breakpoints in templates.
Sometimes I end up with 5 or 6 breakpoints in the same spot.
I've had some success selecting and deleting them in the Breakpoints
window, but like you, I'm looking for a way of make them go away
permanently.
