There is a difference in selecting the file name and selecting the
shown found instance in the file. Selecting the first will AFAIR
replace all in the file.
I'm using 2.4.1. There is an error.
I usually switch over to TextWranger when I want to do some
serious find and replace. Xcode doesn't get all the matches all
the time.
I use BBedit for the heavy lifting, including multi line
replacements etc.
