I noticed the following odd issue: when I edit a toolbar in a nib or xib file using Interface Builder, it appears to revert back to its pre-edited state upon closing the toolbar's associated window (this can be confirmed by reopening the window).

I've had this happen too (though I haven't formally filed a bug report.). I even see the toolbar contents revert without having to close the window first, though I don't know the exact steps to reproduce.
