Given the latest Xcode to support iPhone is still under NDA, no one will be able to directly talk about it here, but IB is used extensively for Cocoa development for the Macintosh. In fact, other than proof of concepts to show you can in fact build a Cocoa GUI without IB, I've never heard of anyone doing that in practice.

OpenOffice.org Aqua version for Mac OS X builds the Cocoa GUI from code and doesn't use IB, other than for a mostly empty MainMenu.nib which no one ever edits.

OpenOffice.org makes fairly extensive use of Objective-C++ with many C++ wrappers around Cocoa objects.
