I removed the .pbxuser file, the .perspective file, and
the .perspective3 file from the project wrapper directory. This
resulted in a 2 button project.

Try and see if you can fix it by removing your *.pbxuser files from
the project wrapper directory.
Failing that, you might have stored a default window layout that
propagates this setting to new projects. You can fix that by
removing this file:
~/.XcodeDefaultPerspective.perspectivev3
