This may sound a little newbish, and not that I expect the menu "Refactor" entry to function any differently than the context menu; but I've found it easier to create the right "environment" for Xcode to do Refactoring when I'm using the context menu entry than the menu bar entry...

Naturaly, YMMV.

When you say "select a symbol", do you mean select the name of the class either in the class declaration, the class declaration, or a usage of the class? Refactoring doesn't work from, say, the Project Symbols smart group, only from a source code file or header. Also, the text editor has to have focus. You can't, say, have selected something in the editor, and then select a group in the Groups & Files tree.
