The precise mechanism is not known, but it seems to happen most frequently in directories where subdirectories have recently been deleted. The presence of the build folder in the project folder is probably what makes Xcode project folders susceptible to this bug; setting the Intermediates and Build Products Location may help you avoid it.

Restarting the Finder or logging out and logging back in again are said to fix this, but it's not known whether those are primary fixes, or simply allow enough time to pass that the primary problem resolves itself while that's happening.
