I use the same Apple ID on five devices.
Mandatory? I don't know. I think in principle an iPhone should run fine without an Apple ID, but I don't remember from the setups whether Apple insists on it.
If it isn't mandatory to have one, it's certainly impractical not to have an iTunes/App Store account (which entails creating an Apple ID). Not using the App Store might be an option for jailbreakers who additionally don't mind piracy, but a developer can't afford to do without.
