AFAIK it is in <yourusername>.pbxuser or <yourusername>.mode? in the .xcodeproj bundle.

I had once  a zombie breakpoint that kept coming back. It turned out to be set not in the current project but in one of the included subprojects. That was, however, in the Xcode 2 time.
