I get this occasionally, not in the iPhone simulator but in a bog-standard
OS X app. My workaround, when I'm at a place where I know it's going to
happen, is to set a breakpoint on the next line and hit Go. It's no
fun living with this but at least it's workable. Sounds like you have the
problem much worse than me though. When it happens to me, CPU usage is
100% while 'step over' is failing to step. I can't recall offhand whether
it's gdb or Xcode which is using all the CPU at that time.

I'm trying to debug an iphone app in simulator, and I set a
breakpoint in my code. when the breakpoint hits- I click "step over" and
the debugger seems to hang. Nothing I do has any respose - trying pause does
nothing, and I can't do basically anything besides stop the app.
This
happens _every time_ on different breakpoint places, and I'm totally stuck
and can't work.
This has been happening to me more or less since xcode
3.1 (where certain breakpoint places would result in this behavior,
others wouldn't), and I'm now on xcode 3.2.3 and the problem only got
worse.
Help ?!?!
