If my app has Portugal and Brazil, but not plain Portuguese, and the user has plain Portuguese at the top of their list, the OS uses the next language in the list rather than automatically trying another Portuguese variant. I'm wondering if I need to worry about this case.

I am answering because I am Brazilian...
I've never heard of regular Portuguese, but if I were you I'd select one of the two other options as regular.
if the user is not satisfied he might try what would seem more appropriate to him/her and I am sure he/she would know where to look.
One reason to select which one is regular would be your market.
