Why are there duplicate entries in the open-files lists?

Hi all.

In every session working with Xcode, duplicate file entries start
piling up in the open-files drop-down list above each editing pane.
They are definitely pointing to the same file, as can be verified with
"reveal in Finder."

This is also sometimes accompanied by failure to save files on
building (I have that option set to "always"), which is a huge
time-waster because (of course) none of the fixes you implement will
be reflected in the executable and you'll wonder why they didn't work.

Has anyone else seen this or have some insight?  I'm using the
all-in-one view in Xcode 3.2.5.  It happens on both machines I use for
development, and has continued through a total wipe and reinstall of
one of them.  My project resides on an external USB drive.
