Jason,
I've had some issues with Xcode 3 not reliably showing you which files have changed, but it sounds like it's impacting you more severely than I've ever experienced. When something doesn't look right, I use the SCM>Refresh Entire Project menu. This seems to re-synch everything, and I tend to find that resolves any problems I was having. I don't recall having to do anything special with the initial setup. Our SVN repository has been in use for years by a much larger Java based project, and I recall connecting Xcode to it to start capturing our iPhone projects, and it just worked.
That being said, if you are at the beginning of your endeavor here, you may want to consider switching to Git. We are about to make that conversion. I think Git offers a number of very useful enhancements over SVN. I have heard but have not tested for myself that Git is a first class citizen in Xcode4.
