OK, I'll answer my own question. My confusion was caused by the fact that the thing you download from the member site does not actually install Xcode, despite being named "InstallXcode.pkg". Rather, it installs an assistant, which contain hidden within its package an Xcode installer. I suppose that the extra levels of indirection have to do with the App Store, even though in this case I was not using the App Store. Anyway, the steps to do a custom install of Xcode 4.1 are:
1. Download and open the .dmg file.
2. Run InstallXcode.pkg.
3. Right-click on /Applications/Install Xcode.app and select Show Package Contents.
4. Find Xcode.mgkg in the Contents/Resources folder and run it.
5. When you get to the right step, you'll be able to select a custom install location.
