I am using Xcode 4.5.2 on Mountain lion. I have a large project (221 source files, 330 artwork, 35 xib) and I don't have indexing issues. So unless your issue is related to Xcode own files, I don't think than reverting to lion or changing versions should work (quick test: move Xcode + preferences on an external hard drive. Download it again).
Could this be a permission problem ?
Creating a new project will tell you if the issue is on Xcode project/workspace config files or in your source files.
