It's been around for a while. I first started running into it in 4.3.
I've been able to minimize the occurrence of the issue through judicious use of cmd-. to stop tasks from xcode, before starting up the simulator again. YMMV.

It's not new; that bug has been in Xcode since 4.0. You and Lance are right that the only out is to force-quit Xcode.
